As a sole trader in digital marketing do I need to worry about having Terms and Conditions?

Yes – for all sorts of reasons, but in particular so that you can seek to limit your liability for losses arising from your breach of your contracts with your customers or other errors arising in relation to your work.  As things stand your potential liability is likely to be unlimited, and as you are a sole trader, all your assets including your home, are potentially at risk if you were sued by any of your customers.  A standard set of terms and conditions, if properly drafted, would limit your potential liability.
